CARE, SHAPED AROUND YOU
Prepare for a care conversation.
Start with everyday life
Think about what is becoming difficult, what is still working well and what the person wants to keep doing independently.
Talk about preferences
Routines, food, language, faith, culture, hobbies and communication preferences can all help shape a personal care plan.
Consider practical arrangements
Discuss the home environment, visit times, access, family contacts and what support is already in place.
Ask about the care plan
Understand what each visit includes, who will provide support, how changes are handled and how to share feedback.
Understand costs
Ask for the proposed costs and what they cover, including any separate charges and the terms for changing arrangements.
Review together
Agree how care will be reviewed, how the person can raise concerns and how others may be involved with their consent.
