We begin by understanding your personal care needs, routines and the home environment. We discuss a suitable carer, their accommodation, agreed working time and rest, meals, responsibilities and how family members will be involved.
Your care plan sets out the support to be provided, what happens during breaks, and any separate overnight arrangements. We agree communication and review arrangements so that care can be reconsidered as your needs change.
